Partnership HealthPlan is a non-profit, community-based health plan operating as an independent public/private partnership across 24 northern California counties. Founded more than 30 years ago by hospitals in Solano County, the organisation contracts with the State of California to administer Medi-Cal benefits through local care providers, currently serving over 950,000 beneficiaries.
Its work spans several areas central to managed care: care coordination, population health management, quality improvement, patient safety, and member navigation. The organisation's Quality Improvement Department works directly with providers across multiple initiatives, and Partnership HealthPlan runs quality incentive programmes designed to raise standards in patient experience, population health, and patient safety. It also supports workforce development and provides resources to help members navigate their healthcare journey.
Partnership HealthPlan collaborates closely with hospitals, providers, and community organisations to improve health outcomes and expand access to care for vulnerable populations. This network-oriented, community-rooted model shapes how the organisation approaches both its operational work and its partnerships across northern California.
